I agree not to hold Connect Sports (DBA as Atlanta Sports Institute) and Welch Elementary, its employees or any staff member liable for any injury that occurs to my child during Connect School Programs.

Late Pick-up Policy

I understand that if my child is picked up after 4:15 PM that they will be placed in the CCSS Afterschool program. The cost associated with this program is $20 registration fee and $12 for the drop-in. I agree that I am responsible for paying these fees if I am not present to pick up my child. I agree that Connect Sports (DBA Atlanta Sports Institute) is not responsible for these fees.
